Stroke triage tools: cost-effective, hard to sustain
Systematic review of prehospital stroke triage tools (mobile stroke units, telemedicine, biomarkers, AI) found most reports of cost-effectiveness positive, but sustained implementation is limited by complexity, workflow fit and costs.

How rare is a prehospital birth? Very
Eight-year cross-sectional review of Danish EMS records found unplanned prehospital births attended by ambulance staff are rare — 310 assisted deliveries, 0.03% of all missions — with most complications manageable by crews.
-
NOF fracture care runs on tacit knowledge
Australian exploratory qualitative study running eight online focus groups with 21 registered paramedics on suspected neck of femur fracture care — participants described managing these patients through diagnostic uncertainty, tacit knowledge, peer learning and contextual judgement in the absence of dedicated prehospital guidance, and called for clearer evidence-informed direction covering movement, dignity, communication and analgesia.
-
Paediatric OHCA: half had warning symptoms
UK single-centre review of 58 consecutive paediatric out-of-hospital VF arrest survivors without a pre-existing diagnosis found 50.9% had reported cardiac symptoms beforehand — syncope in 23.6%, half the arrests during exertion — and of 13 children with prior syncope only three had sought medical attention and none had follow-up arranged, pointing to missed opportunities to diagnose inherited cardiac conditions before arrest.
-
Tabletop MCI exercises: how they're run and measured
Scoping review of 13 studies from 9 countries on tabletop exercises for prehospital mass casualty preparedness — it built a preliminary three-tier 'TTX Design Spectrum' typology and classified outcomes with the Kirkpatrick model (plus an applied-learning level), finding evaluation focused on participant learning with less assessment of applied performance or system outcomes.
-
EMT pass rates vary by program size and region
A US descriptive analysis of 8,567 EMT program cohorts (2018–2022) found median first-attempt certification pass rates of 63.9%, varying by cohort size, region and year, with larger cohorts performing better.
-
Patients report prehospital safety incidents
Structured telephone interviews 15–20 days after EMS transport across eight Spanish provincial services characterised patient safety incidents reported by patients and witnesses, covering near misses and no-harm events as well as harm.
-
Consensus on designing prehospital stroke trials
An expert consensus statement setting out operational, ethical and technological priorities for designing prehospital stroke trials in conventional EMS settings, covering consent models, randomisation, endpoints and statistical frameworks.
-
AI telephone triage protocol for EMS calls
Protocol for the trIAje project, a Spanish retrospective cohort linking 2020–2024 dispatch records, ePCRs and health-registry data to evaluate and then improve AI-assisted telephone triage of time-sensitive conditions using NLP on dispatcher call text.
