Causal model for prehospital blood trials
A Causal Model for Prehospital Blood Transfusion and Mortality in Traumatic Hemorrhagic Shock
Prehospital Emergency Care · Added 22 September 2026
Methodology paper building an expert-refined causal model for prehospital blood transfusion trials in traumatic haemorrhagic shock — maps confounders (system times, training, units available) and mediators so future studies can actually isolate a treatment effect against crystalloid
- Study type
- Modelling
- Design
- Expert-refined causal modelling methodology paper
- Participants
- Not applicable (no patients; multidisciplinary expert stakeholder review)
- Main finding
- Developed a causal model linking prehospital blood transfusion to mortality in traumatic haemorrhagic shock, mapping confounders (system times, clinician training, blood availability), mediators (physiologic and system) and colliders to guide future studies
- Caveat
- Methodological framework only; no patient data or treatment effect estimates
- Bottom line
- Context only
Why it matters in Australia (Medium relevance). transferability — blood products aren't in the extracted ACTAS formulary — would need a guideline change to adopt; QAS, AV, SJWA and NSWA already run prehospital blood programs, so how this evidence matures matters interstate
Learning goal
Strengthen my judgement on which system and clinician factors confound the benefits of prehospital blood in traumatic haemorrhagic shock.

Traumatic arrest in older adults
French national registry study of 5,595 traumatic cardiac arrests comparing older (≥65) with younger adults — older patients achieved ROSC more often (22.5% vs 18.6%) but 30-day survival was lower (0.5% vs 1.2%), leading the authors to argue age alone shouldn't preclude resuscitation though a concurrent medical cause should be sought

Whole blood during prolonged entrapment rescue
Case report of an entrapped trauma patient after a motor vehicle collision receiving advanced airway management, finger thoracostomies and multiple units of low-titre O-positive whole blood during a prolonged multi-agency extrication — the blood came via the regional prehospital program supporting a physician response team

Fluids in traumatic cardiac arrest: rat model
Rat model of traumatic cardiac arrest (30% haemorrhage plus asphyxia) comparing four fluids after ROSC found 96-hour survival of 90% with whole blood, 80% with HES, 60% with lactated Ringer's and just 30% with normal saline — crystalloids prolonged survival without improving haemodynamics.
