MRI profiles of postanoxic status epilepticus

MRI-based quantitative profiling of hypoxic-ischemic brain injury in postanoxic status epilepticus and other EEG patterns
Resuscitation · Added 25 August 2026

Two-center study of 192 postanoxic patients using quantitative MRI found postanoxic status epilepticus exhibited more favorable hypoxic-ischemic brain injury profiles (higher whole-brain ADC values) compared with generalized periodic discharges or malignant EEG patterns.

Study type
Cohort
Design
Retrospective two-centre observational study with quantitative MRI
Participants
192 comatose post-cardiac arrest patients
Main finding
Whole-brain ADC values were lower in malignant patterns and GPDs (838 × 10⁻⁶ mm²/s) than in postanoxic status epilepticus (893 × 10⁻⁶ mm²/s) and benign patterns (907 × 10⁻⁶ mm²/s); 30.7% recovered from coma
Caveat
Retrospective, in-hospital ICU population, no outcome effect confirmed
Bottom line
Context only

Why it matters in Australia (Medium relevance). Advanced neuroprognostication research — hospital-based MRI study not applicable to prehospital care

Learning goal

Refine my prognostication judgement on EEG patterns after cardiac arrest without prematurely declaring futility in postanoxic status epilepticus.
