Prehospital TXA helps fibrinolytic shutdown
Pooled analysis of two prospective prehospital TXA trials found fibrinolytic shutdown carried 14.5% 30-day mortality versus 6.5% with physiological fibrinolysis, and shutdown patients randomised to TXA had about a third the risk of death.

Opioid OHCA outcomes similar to other arrests
Analysis of 10,786 EMS-treated OHCAs in a Canadian registry found opioid-associated arrests had similar outcomes to undifferentiated arrests (OR 1.02), but Utstein variables like initial rhythm and witnessed status predicted outcome differently in the opioid group.

Three post-ROSC lab phenotypes predict outcome
Latent class analysis of 14 laboratory values drawn within six hours of ROSC in 1,514 resuscitated OHCA patients identified three phenotypes, with the multi-organ dysfunction group at roughly double the odds of poor neurological recovery compared with the others.

Prehospital variables can't pick pelvic binder benefit
Blinded imaging review of 298 CT-confirmed pelvic ring injuries found binder application would be potentially mechanically unfavourable in 56% of cases, and no prehospital variable (hypotension, shock index, mechanism, GCS) discriminated favourable morphology well.
