TOR rules uncertain in choking-related OHCA
Performance of termination-of-resuscitation rules in foreign body airway obstruction-related out-of-hospital cardiac arrest: a prospective multicenter registry analysis
Prehospital Emergency Care · Added 27 July 2026
In FBAO-related OHCA, BLS-TOR would have classified two patients with favorable neurological outcome as TOR-positive.
- Study type
- Registry
- Design
- Prospective multicentre registry secondary analysis
- Participants
- 248 patients with foreign body airway obstruction-related OHCA (25 Japanese hospitals)
- Main finding
- BLS-TOR classified 150/229 evaluable patients as TOR-positive, of whom 2 (1.3%) achieved favourable neurological outcome (specificity 77.8%, 95% CI 45.3-93.7); ALS-TOR classified 12/242 as TOR-positive with no favourable outcomes but 4 (33.3%) survived to 30 days
- Caveat
- Very small numbers of survivors, precluding firm safety conclusions
- Bottom line
- Context only
Learning goal
Strengthen my judgement on when termination-of-resuscitation rules can be safely applied in choking-related cardiac arrest.
