SCARE score validates for chest pain triage
External Validation of the SCARE Score for Predicting Acute Myocardial Infarction in Patients Calling Emergency Medical Communication Centers: A Prospective Multicenter Study
Prehospital Emergency Care · Added 4 June 2026
They prospectively validated the SCARE score for predicting acute myocardial infarction during telephone triage across multiple French emergency communication centers, finding good discrimination and calibration but noting limitations in real-world implementation.
- Study type
- Cohort
- Design
- Prospective multicentre observational validation cohort
- Participants
- 2,042 adults hospitalised after EMCC calls for non-traumatic chest pain
- Main finding
- 265 (13.0%) had AMI; SCARE AUROC 0.85 (95% CI 0.82-0.87), AUROC 0.84 in men and 0.89 in women; at >35 points sensitivity 73.6%, specificity 75.3%, NPV 95.0%
- Caveat
- Score not used in real time and did not influence dispatch; French physician-staffed dispatch centres, so applicability to non-physician dispatch systems untested
- Bottom line
- Hypothesis-generating
Learning goal
Weigh how structured chest pain telephone triage tools could strengthen my recognition of acute myocardial infarction risk.

PANDA trial: noradrenaline vs adrenaline in shock
This multicenter RCT protocol tests whether noradrenaline improves outcomes compared with adrenaline for paramedic management of cardiogenic shock, with the PANDA trial aiming to settle the debate on optimal vasopressor choice in the prehospital setting.
